Team Check In

All Check In for the Halloween Classic is done electronically through GotSoccer.

Check In must be completed by Friday, October 18th.

How to Register:

  1. Log into your GotSoccer Team Account and select Halloween Classic under Event Registration History.
  2. Click Documents Tab and upload the following documents (PDF Format)
    1. Official Team Roster (stamped and approved by your State Association)
    2. Player and Coach cards (every participating player/coach)
    3. Medical Release Waiver (1 document- every participant listed/signed)
    4. Travel Permit (only for teams outside of Indiana)

Documents

All teams are responsible to upload the folloiwng documnets to their GotSoccer account:

Official Team Roster - (Tournament Keeps A Copy)

  • Must be an Official Roster signed by a governing body official (registrar).This signature is often in stamp form or printed in a different color
  • All players not participating should be marked out with a Pen or Marker
  • Guest Players - Please write the anmes on the copy of the official roster. All Guest Players must have a player pass and medical release form.

Player/Coach Passes -

  • Must be Official Player/Coach Cards - No Photocopies will be accepted.
  • Must be Laminated.
  • All participating players and coaches must have an official card.
  • All cards and the matching roster must be from the same governing organization (USYS or US Club).

Medical Release Form - Must be completed for all participants.

Team Medical Waiver

Permission to Travel - (Out of State Teams Only)

  • All teams traveling from out of state must submit a Permission to Travel from their state association.
  • US Club soccer teams are not required to have travel permits
